What this notice usually means
LDSS-3174 is New York's Recertification Form for Certain Benefits and Services. The state agency OTDA sends it when you renew SNAP along with Public Assistance or Medicaid at the same time. If you only get SNAP, your packet may show a different form, called LDSS-4826. OTDA mails your packet about two months before your certification period ends. It lists your interview date and time. Your own packet has your real form number and dates. OTDA decides your case.

What to do now

Check your packet for your form and interview date
OTDA mails your recertification packet about two months before your certification period ends. It lists your scheduled phone interview date and time. Find your real dates on your own packet, not on this page.

Submit your recertification before your interview
Send your form in before OTDA interviews you. If you send it late, call to change your interview date. Do this before your certification period ends.

Send any extra proof by your new deadline
If OTDA asks for more documents after your interview, a separate notice will tell you what is still needed. You get at least 10 days to send it. Sending proof late can reduce, delay, or deny your SNAP benefits.

Watch for your approval or denial
OTDA must decide your case within 30 days of your filing date. They will tell you if it is approved or denied. If your income and savings are very low, you may get benefits back within 5 days.

Call your local district if anything is unclear
Use the Call Request Time Form in your packet to change your interview time. Or call the number on your own notice.
