Colorado does not fit the classic MCO-roster frame: most Health First Colorado members are assigned a Regional Accountable Entity (RAE) for care coordination, not a plan they pick from a list. This file therefore models only the two genuine plan-choice areas Colorado actually publishes, per the state's own Accountable Care Collaborative FAQ (captured at co-hcpf-acc-faqs.txt): Elevate (Denver Health) Medicaid Choice in Denver, Adams, Arapahoe, and Jefferson Counties, and Rocky Mountain Health Plans (RMHP) PRIME in Delta, Garfield, Gunnison, Mesa, Montrose, Ouray, Pitkin, Rio Blanco, and San Miguel Counties. Colorado's Preferred Drug List is fee-for-service only and explicitly does not apply to either MCO (hcpf.colorado.gov/pharmacy-resources, cited informally, not fact-gated), so each plan's formulary_url points at the plan's own pharmacy page rather than a shared statewide URL, unlike CA/TX. Per-plan member_phone, provider_directory_url, formulary_url, and extra_benefits were independently verified live on each plan's own site (Denver Health Medical Plan's Elevate Medicaid Choice pages; UnitedHealthcare Community Plan / RMHP's PRIME pages) but are not fact-cited, the same standing as every other state file in this corpus. Elevate Medicaid Choice's own site lists two different phone numbers in different places (303-602-2116 / toll-free 1-800-700-8140 near the top intake widget, versus 303-602-2116 / toll-free 1-855-281-2418 in the page's own accessibility-help footer); this record uses 1-855-281-2418 because it matches the number HCPF's own ACC Phase III contract table publishes for this MCO, and that discrepancy is noted on the plan record itself. RMHP PRIME's own site did not surface a distinct value-added-benefits page beyond standard Health First Colorado covered services, so its extra_benefits is limited to the one benefit (CirrusMD telehealth) explicitly described as offered by RMHP at no cost, rather than padding the list to match Elevate's longer one.
