Colorado does not fit the classic MCO-roster frame: most Health First Colorado members are assigned a Regional Accountable Entity (RAE) for care coordination, not a plan they pick from a list. This file therefore models only the two genuine plan-choice areas Colorado actually publishes, per the state's own Accountable Care Collaborative FAQ (captured at co-hcpf-acc-faqs.txt): Elevate (Denver Health) Medicaid Choice in Denver, Adams, Arapahoe, and Jefferson Counties, and Rocky Mountain Health Plans (RMHP) PRIME in Delta, Garfield, Gunnison, Mesa, Montrose, Ouray, Pitkin, Rio Blanco, and San Miguel Counties.
