You can request a new IRMAA determination only if you experience a specific defined life changing event, such as marriage, divorce, death of a spouse, work stoppage or a reduction in work hours, or certain losses of income producing property or pension income. A general drop in income by itself does not qualify on its own.
What happened
A Medicare premium surcharge called IRMAA uses income from an earlier tax year. This is often two years back. Income may have dropped since then. That older number can feel out of date. There is a federal process for this. You can ask Social Security to use newer information instead.
What usually applies
You can ask for a new IRMAA decision only after a specific life event. This list includes marriage, divorce, or the death of a spouse. It also includes stopping work or cutting your hours. It includes losing income producing property too. It includes an employer pension plan ending. It includes a settlement tied to an employer closing or going bankrupt. A one time income event with no life event behind it does not count on its own. This includes a single large Roth conversion.

Check whether your situation matches a listed life changing event
Retirement often qualifies through stopping work or cutting hours. It must be tied to a real work change. A lower income year alone is not enough.

Ask Social Security about Form SSA-44
Use this form to report your life event. It asks Social Security for a new IRMAA decision based on newer income.

Gather proof of the life changing event
Bring documents that prove your event. This can be a retirement letter, a divorce decree, or a pension notice.

Ask what income year Social Security will use instead
A successful request swaps in a newer income estimate. This replaces the older tax year Social Security first used.
