What this notice usually means
Form SSA-44 asks Social Security to lower a Medicare premium surcharge. You file it after a life event, like retiring, that dropped your income. Using the form is optional, but it can help avoid waiting for new tax data.
What to do now

Form SSA-44 lists the accepted events, such as retirement, divorce, or the loss of income producing property. Check your own event against that list.
- 2
Gather proof of your income change
Collect a retirement letter, a signed statement, or other proof. It should show the income drop and when it happened.
- 3
Submit Form SSA-44 through a listed channel
File the form online, by mail, or at a local Social Security office. Your proof.
- 4
Call Social Security if you have questions
Call Social Security at 1-800-772-1213 (TTY 1-800-325-0778) to ask which event and proof fit your situation.
