What this notice usually means
An HHCCN tells you about one change in your home care. Your home health agency gives it when reducing or ending one service. It is not the same as ending all of your home health care.

Your HHCCN names the specific item or service being reduced or ended, and the reason given.

Ask how this affects your overall care plan
Ask your home health agency whether other services in your plan of care continue as before.

Keep a copy of the notice
Keep your copy of the HHCCN, since it matters later if you have questions about your care or coverage.

Call your agency or Medicare with questions
Ask your home health agency to explain the notice, or call 1-800-MEDICARE (1-800-633-4227).
