Form Numbers on Government Letters

Most letters have a number in a corner. Type it above, or pick the family it belongs to.

DE forms (California EDD)

DE forms come from the Employment Development Department in California, or EDD. They cover unemployment claims, payments, and appeals. The DE number is printed near a corner of the page.
