This file was expanded 2026-07-17 from 2 to all 58 California counties, grouped into 24 roster-distinct service areas. Every area's plan roster is quote-gate verified: 12 areas (the DHCS Health Care Options 'compare plans' choice counties: Los Angeles, Sacramento, San Diego, San Francisco, Santa Clara, Kern, El Dorado, Alpine, the Amador/Tulare pair, the Calaveras/Inyo/Mono/Tuolumne regional-model group, the Fresno/Kings/Madera CalViva group, and the Riverside/San Bernardino Inland Empire group) cite a live-fetched county page from that tool. The other 12 areas (single-plan/COHS counties the compare tool returns empty for, since it only renders 'choice' counties) cite DHCS's own statewide 'Medi-Cal Managed Care Plans by County (as of 2023 and 2024)' PDF (dhcs.ca.gov), corroborated where overlapping with the live tool's data (Sacramento's 2024 roster matches; LA's Health Net/Molina subcontract note matches). All 58 county pages were fetched headless and sha256-snapshotted; only the 13 actually cited by a fact are declared as sources[] here per the bloat guard, but every county snapshot file remains on disk at seed/plans/snapshots/CA/. enrollment.portal_url, enrollment.portal_name, and enrollment.phone (1-800-430-4263, found on HCO's own /en/contact-us page, not the homepage) are quote-gate verified. enrollment.choice_window_days is still an honest not_found (DHCS's own pages never state an exact day count in captured text). Plan records: consolidated the prior prototype's 7 county-scoped duplicate records (e.g. 'Kaiser Permanente Medi-Cal, Los Angeles' and '..., Sacramento') into single statewide records per the frozen spec's explicit Kaiser instruction, since Health Net, Anthem, Molina, and Kaiser all show the identical statewide member phone number on every county page checked; L.A. Care kept its original id (la-care-medi-cal). All 23 plan records have a real member phone, provider directory URL, and the shared statewide Medi-Cal Rx formulary URL (medi-calrx.dhcs.ca.gov). 14 of the 23 plans' phone/TTY/website were read directly off DHCS's own comparison tool (tier 1); the remaining 9 (mostly single-county COHS plans the tool never renders) plus every plan's extra_benefits and provider_directory_url came from the plan's own site via parallel research agents, none of it quote-gated against a snapshot, so verified stays false for the whole file. Two research-flagged soft spots: Molina Healthcare of California's site actively blocks automated fetches (its phone number is still HCO-tier-1-verified, but its provider_directory_url and 'no extra benefits found' status are unconfirmed by direct fetch); Gold Coast Health Plan and Contra Costa Health Plan both 403 automated fetches (goldcoasthealthplan.org, cchealth.org), so those two plans' extra_benefits/URLs are corroborated via cached/indexed snapshots of their own domain rather than a live fetch. NCQA ratings stay null for all 23 plans (unverifiable). No county's roster fetch failed outright: all 58 HCO county pages returned 200 with real content (a 'no county selected' empty state for the 37 non-choice counties, or a real plan roster for the 21 choice counties).
