What this notice usually means
Form SSA-561 is Social Security's first appeal form. You use it to ask for reconsideration after a denial. Social Security reviews the case again and can change the earlier decision.

Your denial letter explains the reason and the deadline to ask for reconsideration. Keep it with your SSA-561 form.

Gather updated records
Collect medical records, work history, or other proof. It should connect to the reason SSA gave for the denial.

File Form SSA-561 through a listed channel
Submit the form online, by mail, or at a local office. Follow the instructions on your denial notice.

Call Social Security or get free help
Call Social Security at 1-800-772-1213 (TTY 1-800-325-0778). A local legal aid group can also help for free.
